Medihub is here to help.
If you have recently been discharged from hospital, and are unsure about your new medication – we are here to help. Here at Medihub, we offer a Discharge Medicines Review in our Pontarddulais and Killay stores to customers following their return from hospital. If your medication has changed between admission and discharge from hospital, our Pharmacists and Pharmacy Technicians will be able to support you through these changes.
Our highly-trained team members will work with your GP practice, to obtain any new medication items prescribed, discuss with you when and how to take and be on hand to discuss how these changes may affect you, or discuss any issues or concerns that you may have.
